The form used to report a collision to the DMV is:
- The SR 22 insurance certificate
- The police report filed by the officer
- The SR 1, Report of Traffic Accident Occurring in California — correct answer
Why
Each driver files an SR 1 at dmv.ca.gov/accidentreport. The SR 22 is a different document: proof of insurance. (Section 10, Reporting a Collision.)
Practise this in the real format
Reading one answer is not the same as sitting the test. The simulator runs the first-time adult form: 36 questions, 6 mistakes allowed, 83% to pass.
